Monitor your monitoring

As soon as you end a visit with a member, the clock starts ticking.

As service coordinators, we are obligated to see our “frail” and “at risk” members every 30 days. Those who are not “frail” or “at risk” must be seen every three months. This means that if we see a frail member on August 13, our next appointment should be scheduled before September 13.

The easiest way that I’ve found to keep track of client monitoring is by creating a simple spreadsheet. My spreadsheet consists of several different tabs that include various different lists.

You can create a tab in an Excel file by going to the very bottom of the spreadsheet. There, you can right click and voila! Rename the sheet to suit your needs.

To keep track of visits and contact with my “frail” and “at risk” clients then, I run a simple list in Excel. On the left side of the list are names, on the right side — you guessed it — the date when I had last seen him or her.

To some, Excel can be rather intimidating. However, in this instance, the program makes it pretty easy to track clients. In Excel, you can highlight names as well as add and delete entire rows. This can be particularly helpful as members become deactivated or when new members move in.

Additionally, you can run several different lists using Excel. My lists include: Enrolled Members, Active Members with Non-Participation Forms, Active Members who are Frail and At Risk, Active Members who are not Frail and At Risk.

At the very bottom of this post, you will find the word “Example” in blue. This is a link to a sample spreadsheet. If you are not familiar with Excel, please feel free to open the link (or open Excel on your hard drive) and play around with it a bit. It is quite a simple program, once you get used it.

By keeping these running lists, I know who’s who at all times — and so far, it’s worked for me!

Announcing 2 New SC Team Members!

Welcome Jane Grant and Matt Hamburg to the Hannan Service Coordination Team. They started today! Both Jane and Matt are social workers. Jane brings a career’s worth of experience as a Detroit social worker accross the life span to her full time position at Meyers Plaza Co-op. Matt interned at Hannan for a full year and graduated from UM SSW two weeks ago. He’ll be part time at Belleville Co-op and part time “special projects”, which translates to long term substitute service coordinator where needed. The first place he’s needed is at Wyandotte Co-op.

CSI Annual Management Meeting and Luncheon

The Council members at my co-op have invited me to the CSI Annual Meeting on 9/28 and offered me one of their co-op guest tickets. If it is appropriate, I would like to attend. Do you happen to know what CSI’s stance is on SCs attending this event?

Sharon’s Answer:

You would be attending the Annual Meeting Luncheon as a guest. The cost of the ticket is $15. If the co-op is paying for your ticket it should be approved by the council since the payment will come from their house funds (fundraising profits).

 

CSI employees that are not members of the co-op (purchased a $100 non-housing membership) are required to take a vacation day and purchase a ticket. I just spoke with Estrelita our H.R. person and the hours for this day are not covered for our non-members CSI employees to attend. They must take a vacation day. Therefore, if a SC attends he/she will need to use a vacation day. CSI will not bill this as a working day.

Works of David Rubello

Reception to meet the artist

Native Detroit artist David Rubello studied art at Cass Technical High School and The Society of Arts and Crafts (now known as the College  for Creative Studies). A keen interest in art history enticed him to study at The Academy of Fine Art in Rome, Italy  and at The Kings Academy of Fine Art in Copenhagen, Denmark. Further study at University of Michigan in Ann Arbor culminated in a Master of Fine Arts degree.

As a professor of art, Rubello has taught painting and drawing at University of Michigan,  Pennsylvania State University, Towson State University, Maryland, Macomb Community College, College for Creative Studies,  and  Daytona State College.

Invited to exhibit his work in both national and international exhibitions, David Rubello’s art works are included in many public and private collections. His relief paintings have been published in several issues of The Structurist Journal and his fine art photography was featured in Black and White Magazine.

Gallery  hours are Monday – Thursday by appointment, and Fridays 9 am-7pm. The Gallery is closed weekends.

Caregiver Support Group at PVM Westland

A new Caregiver Support Group is forming at the Village of Westland… please join us!
The next group meeting is Thursday August 16th at 6:00 p.m. The group will continue to meet on the 3rd Thursday of the month. Facilitated by Nancy Coman of Wayne Metro Community Action Agency, this is an outlet for caregivers to gather, know they are not alone, and share helpful insights and tips for caregiving.

Fair Housing Act Expands to prohibit discrimination on the basis of sexual orientation, gender identity and marital status

The Fair Housing Act prohibits discrimination in rental, sales and lending of housing on the basis of color, disability, familial status, national origin, race, religion and sex. In addition, the U.S. Department of Housing and Urban Development (HUD) has new regulations intended to ensure that HUD’s housing programs are open to all eligible persons without regard to their:

Sexual orientation

Gender identity or

Marital status

HUD’s goal is to ensure that their programs are carried out free from discrimination and are models for equal housing opportunity. Now lesbian, gay, bisexual, and transgender (LGBT) individuals are guaranteed equal access to HUD housing and shelters.

The new regulations were published in the Federal Register as

Equal Access to Housing in HUD Programs – Regardless of Sexual Orientation or Gender Identity. The rule took effect on March 5, 2012 and it makes the following provisions:

1. Requires owners and operators of HUD-assisted housing, or housing whose financing is insured by HUD, to make housing available without regard to the of sexual orientation, gender identity, or marital status.

2. Clarifies that all otherwise eligible families, regardless of marital status, sexual orientation, or gender identity, will have the opportunity to participate in HUD programs.

3. Prohibits owners and operators of HUD-assisted housing or housing insured by HUD from asking about an applicant or occupant’s sexual orientation and gender identity for the purpose of determining eligibility or otherwise making housing available.

Fair housing practices is one of CSI’s corporate core values. At CSI, we embrace the diversity of our members and treat all members with respect. We encourage our members to recognize the value of their diverse communities. We are consistent in our standards, policies, and the treatment of our members to promote fairness and equality.

Fall Prevention by Dr. Allon Goldberg

Anne Sackrison confirmed that having a Professor from WSU conduct a workshop that included an optional referral to a University-sponsored study is not solicitation. Dr. Goldberg and his students have provided numerous fall prevention activities at the Hannan Center for Senior Learning over the years, and we also feel comfortable with him and his workshops.

Maryanette’s Liaison had posed this question to Anne after Maryanette shared the workshop idea and potential concern with him:

“The service coordinators from Wyandotte are planning the September education meeting.  They have asked Dr. Allan Goldberg to speak from Wayne State University school of gerontology about fall prevention.  He asked if it would be OK to mention a study that they are currently doing on seniors, which would pay them to participate if they were interested.

I think it would be fine since its through Wayne State.  I just wanted to get your opinion on it.

Hosting a Co-op Health Fair

Information on hosting a health fair is limited and is on the logic model that each of you has for your site. You have a hard copy in your binder and it is available on the shared drive under Program Staff-Service Coordination-Site Specific (select your co-op)- Logic Model. It’s a large excel file. Open the “Year 1” tab. The last outcome/output requested in column J is:

“Policy Priority-Housing as a Platform-Number of HUD housing and community residents participating in periodic mobile health screening events  |Participants”

In order to comply with Hannan and co-op procedures, please follow the following guidelines when hosting health fair:

1. Utilize non-profit and/or governmental services (no for-profits)
2. You can provide vaccination education, but no vaccinations of any kind.
3. Check with Sharon (cc Rachel) on which neighboring co-ops you could invite in order to meet the “community” requirement. This is easy for Madison Heights – just invite New Horizons. For others, Sharon may be able to give guidance. I’m cc’ing Sharon.

Please bring your hard copies of your logic models with you to Thursday’s training here at Hannan. It will help you as Wendy shares the amazing, beastly tool she has created to help you understand and fill out your respective logic models.

Participants must be registered before attending classes.
