Age in Place with PACE presentation
PACE will open their doors in Kalamazoo Co. in March! A presentation to introduce members and their family to the program will be given by PACE. It will describe the benefits of the program, eligibility requirements, and all the modalities designed to assist frail members to to get the help they need to keep them in their homes. This resource is amazing… From hot meals to in home nurses, to medication to social activities, and usually it’s free of charge!

Healthy Choices, Healthy Living at Flat Rock Towers!

Diane Crawford and Victoria Haltom, service coordinators at LaBelle Tower, agreed to lend a hand at Flat Rock Towers! This month, Crawford and Haltom are presenting their Healthy Choices, Healthy Living workshop at FRT. Thanks for helping me out!

PATH Workshop begins at Flat Rock Towers

PATH (Personal Action Toward Health) is a 6-week workshop developed by Stanford Univeristy and offered by The Senior Alliance. Recently, Lindsay, Andrea, and myself all attended PATH training at TSA. Yesterday, Flat Rock Towers had it’s first PATH workshop, which was facilitated by Amanda Pier of The Senior Alliance and myself. Please see the attached flyer.
In addition to the flyer, members at FRT also recieved special invitations to attend the event. I created the invitations using PowerPoint. Please see the attached invitations.

1. CSI Update (Rachel)

2. PVM update (Brenda)
There is one SC opening at Oakwood Manor; interviews start next week
3. Finding Joy in the Workplace (Mary Annette)
Based on a webinar that MaryAnnette recently viewed:
– Reflected instances of joy in the workplace when not expecting it
– Shared “jump up and down moments”
4. Client Scenarios:
– Harriet asked a question about background checks for care providers; resources were shared (PHI, MCQ3)
– How are other SC approaching members with hygiene problems? (Victoria)
-education about aging and hygiene concerns
– clients may not be aware of poor hygiene
– integrate questions as part of assessment
– basic informational session: “hygiene on a budget”- Victoria will research this topic
5. Question about recording vetting efforts (Karen)
– sheet to fill out in Ann’s vetting packet from professional development training is available on shared drive
– will help members make more informed decisions
6. Tax deductions and City of Detroit (Karen)
– SCs may have to pay Detroit taxes even if they work at a site outside of Detroit because they are paid by Hannan, which is in Detroit
– Michelle/HR Department to follow up on this
7. Deactivating clients (Rachel)
– part of case management procedures: remove any extraneous/supporting documents
– best practices to black out member’s social security number and Medicare number
– write a last progress note indicating that a deactivation form is being completed
– for a deactivated client who contacts SC after the case has been deactivated, SC can reactivate, add the progress note(s), and then deactivate again (will create a total of two deactivation forms in member’s file)
8. Training new SCs: What “initial” training do current SCs want a refresher in? (Rachel)
– tentative dates for new SC training are week of 3/11/13
– new SC official start date is tentatively set for 3/8/13
9. Direction on what SCs would like to learn from substance abuse training in April (Ann)

MI CHOICE MEDICAID WAIVER
MI Choice is a home & community-based care option for persons who are Medicaid Waiver eligible for their long term care. This program provides a wide range of home & community based services coordinated by a care manager. Eligibility requirements are:
* Age 65+ or, a disabled person age 18+
* Financial eligibility
* Medical eligibility for nursing home level of care
* Have multiple medical & service needs.
This program may provide: Chore services; Personal care; Respite Care; Home Delivered Meals; Non-Medical transportation; Personal Emergency Response System; Counseling services; Private Duty nursing; Training for caregivers.
Complaints about Logisticare
Are your clients having problems with Logisticare? Please encourage them to complain, or ask if you may do so on their behalf. Complaints can be called in to Logisticare at 866-569-1903 or to the state DHS beneficiary line (you have to go through several menu options first) at 800-642-3195. If calling the state, it helps to have the Medicaid ID number of the person affected.
