Education Workshop Topics on Nutrition
Hazel Park Manor is in the process of holding a series of 4 nutritional workshops with a dietitian from the Oakland County Health Division/MSU Extension:
November 7th, 2012 at 2 pm: “Eating Well When Not Feeling Well”- ideas for nutrition related to physical health concerns.
December 5th, 2012 at 2 pm: “Spice Up Your Meals Without Salt”
Recent past workshops included our September event, “Eating Well on a Shoestring.” In October, our presenter spoke about “Meals in Minutes” – ideas about how to make quick and nutritious meals with what is available in the refrigerator and pantry.

Writing Workshop – Optional
Hi All,
Yesterday we discussed hosting an optional peer-supported writing workshop at Hannan prior to the Disabilty Workshop and Vendor Fair on November 13th. The writing workshop is scheduled in Lower Level B (the Berry Room accross from the kitchen in the basement) from 9-11am. The Disability Workshop follows directly from 11am-1pm in Lower Level A (the big room).
Things to consider regarding the writing workshop:
1. It is optional, but I’ll take a poll on our November 8th Conference Call to develop a list of attendees.
2. This is an opportunity to “shadow” each others writing techniques, broadening your skills and methodology for writing professional documents.
3. Writing samples from the workshops can become “templates” for your own reference.
4. Try to bring 3 (or more) writing samples to share in small groups. Think broadly about what you would like to learn and see from others, and then share your own. Ideas that have been shared with me include how to write an Initial Service Plan, general Progress Notes, a letter to a client, a letter to a family member, an educational event flyer, an offer of services to a member, a meeting agenda…..
5. We’ll devide into small teams to read and critique each other’s writing in an effort to expand our knowledge of writing techniques.
6. Please black out any identifying information in the writing samples you bring.
7. This event is optional and open to all Service Coordinators.

Belleville Educational Event
The Belleville office of Service Coordination Presents: Trisha Worthington, Assistant Prosecuting Attorney for the Wayne Co. Prosecuting Attorney’s Office, Elder Law Division. Ms. Worthington will be at the Belleville Co-op on Thursday, October 25 at 1pm to discuss the topics of Fraud and Identity Theft with the members. Any interested SCs are welcome to attend the presentation.

Confused About Veteran Pension Benefits?
Do you receive questions about VA benefits? If so, check out this new fact sheet, recently released by the Veterans Benefits Administration.
The fact sheet explains the different types of veteran pension benefits and who may be eligible. The VA’s pension program provides monthly benefit payments to certain wartime Veterans with financial need, and their survivors.
Semi Annual reporting of members under 62 who are frail or at risk
Answer:
I’m unsure at this point. I have some messages into HUD on this. In the meantime, Laura simply produced a quick memo indicating the number of frail and at risk members below the age of 62 with whom she is working. I think this is a good solution. I’m cc’ing the other CSI SCs so that they can utilize the same technique until we hear otherwise from HUD.

Since our Co-op has Members under 62 years of age, what additional information do we need to add in the report? It was mentioned we needed a memo as well. What do you want included in the memo?

Notes from CSI Conference call on 10/16/12
Phone conference notes for Tuesday, October 16, 2012:
1. Ideas about how to keep an initial service plan concise and thorough.
- Think about the who, what, where, when, and what’s going on
- Think about short- and long-term goals for the member
- Mention the reason for the referral/primary reason for visit, challenges with ADLs, and identify next steps for SC and member
2. Thoughts/questions about the HUD Semi-Annual Report- Anecdotes vs. “trends.”
- Submit one semi-annual report per SC; one logic model per co-op
- Logic model will be submitted in an alternate method; SCs can get started on the logic model by hand
- Anecdotes are fine to submit; please ensure confidentiality. If you have questions about this, Rachel is happy to discuss with you.
- Trends are also appropriate. If you are entering a trend, the entry date should be the last day of the reporting period (9/28/12).
- Snail mail the printed report to HUD
3. Questions about taking paid time off (PTO), sick time, and SC coverage.
- For PTO for SCs with multiple SCs on site please discuss with your fellow SC to maintain 1 SC per co-op, when possible, prior to submitting PTO request form to Rachel.
- For solitary SCs, submit PTO request form to Rachel
- Substitute SC coverage is necessary for SCs who plan to be out of the office longer than 1 week.
- Please let your Council and Liaison know if your scheduled vacation time.
- PTO and sick time are pooled in one bank. In the event that an SC is sick, text or call Rachel at the beginning of the day and retroactively submit your PTO request form once you return to the office.
