Cost Savings Data from Detroit Economic Security Center

Hi All,
Kari asked this morning about the estimated costs savings for helping a member apply for Medicaid. According to the Detroit Economic Security Service Center, the estimated annual savings is $2,495 (or $207/month). You can find this information in the cost savings data sheet below.

Documenting Client Request to End SC Services

Question:

Cheryl, a SC has a client who has requested to withdraw from the SC program. Our procedures do not have a methodology for handling this type of request because once a member is a client, she/he remains a client until she/he moves out or passes away. How do you advise addressing this situation in the case files?

Answer:

I think that you can do the following and our revised procedures in the future need to include.  I think you can do a non-participation form initialed by at least you but better if also is signed by the member and attach her letter.  Then note in progress notes that the case is being taken out of open status and returned to enrollment status only.  I think that you can also do a deactivation form on a member and then do a new enrollment only form and progress notes including the old case notes.  However, Pangea doesn’t like this method, think all records still stay there but not sure. I am not sure how it affects the semi annual client numbers.   For the time being, do the non participation form and include in your enrollment only cases.  Cheryl

Submitting Semi Annual Reports

Hi All,

Please submit your semi-annual reports via snail mail to Sandra Jacques at HUD at:

Detroit Multifamily Hub
477 Michigan Avenue, 16th Floor
Detroit, MI 48226

You should then submit your reports electronically to your supervisor and administrator (Rachel Jacobsen and Sharon Bolden for the CSI SCs).

Please remember who your audience is when writing your reports. It includes CSI or PVM as well as HUD.

Update on Logic Models for CSI SCs from Sandra Jacques. She reports that they are having difficulty with the contractor that holds the Logic Models on their sharepoint site.  We have not been able to get them opened for updating. She has asked us to complete another Logic Model Form and update with the program accomplishments so far and email it to her. I have not received this form from Sandra Jacques yet, so please start by filling out your original form out in pencil so that when we do get the new forms, you just need to type in the information.

Large print resources

Pangea did not know of a way to enlarge the print so I can give large print  AASC resources to members.   After some trial and error, I also made my resource list for the co-op in large print, and listed by catagory.  Here is what I eventually figured out…Clearly I am no genius on the computer! :0 )

Click on the resource you want to hand out, then on Print, Preferences, Effects, click on percentage of normal size and change the percentage (I like 125% ), Click on Finishing, Landscape, OK, and finally Print.

Should Home Health Aide be present for initial interview?

Question:

When we complete preliminary paperwork with members (Intake, AS, ADL, etc…), this is to be only done with the member and the SC, correct? I have a member that would prefer to have her Home Health Aide present during this time (signed consent form on file). According to her Home Health Aide, she has severe cognitive issues and may not be able to answer various questions accurately. I was under the impression that home health aides cannot be present, but when I looked through my previously weekly meetings notes I couldn’t find this information.

Answer:

They can be present with member consent. I would obtain this consent when the home health aide is not present so that the member doesn’t feel pressure from the aide to consent. Go with your gut on their relationship. If the member truly has cognitive challenges, I would recommend exploring someone other than the aide to be present. Th aide is paid and provides vital services to the member, so there is some power in the relationship.

Cheryl Bukoff received information that the Holiday Hope for Seniors gifts from American House Foundation are worth $50-75. Perhaps you should use an average value of $62.50 when accounting for Cost Savings for recipients.

Posting your AASC Online Agency List

AASC Online tip of the day!:

You have the resources your residents/members need at your fingertips. In the upper right-hand corner of your AASC Online Resource Director is the option to Print Agency List. Do it! Then post it on your bulletin board and share it with your residents/members. CSI SCs should bring their FCRC Chairs updates of the Agency list at their monthly meetings for the FCRC Chair to put in their resource binder…..and let your auditor know that you have done this and where the FCRC binder is located. The idea is knowledge is power! Some residents/members would prefer to check out your resource list on their own terms during non-office hours, so make it available. Thanks!

Logging weekly supervision conference calls or meetings

Question:

How do we log our time for the conference calls — should we consider that administrative time?

Answer:

Log them in the SC Daily Log under “meetings” towards the bottom (not the “administrative meetings” at the top – those are for meeting with property management/co-op leadership).
