ACA Volunteer of the Year Award accepted by SC Ting Chang

I’m pleased to pass on the wonderful news that Ting Chang, part time Service Coordinator at Madison Heights Co-op, accepted the Volunteer of the Year award from the Association of Chinese Americans at their 40th Anniversary Celebration.

Co-Service Coordinators Harriet Harris, Kari Wilson, and Katherine Simpson were present at the ceremony representing Hannan and ACA.

Ting and Katherine work for ACA and assist Hannan with 8 hours of bilingual Mandarin/English service coordination at Madison Heights Co-op. Their contributions to our program are vital to working with the members of Madison Heights Co-op, 30% of whom are Chinese American.

Way to go Ting!

CSI Logic Model Reports to HUD

There has been some confusion on the Logic Models. You will be unable to electronically open the logic models you were given. HUD has been unable to unlock them. Hence, we are proceeding by printing out the logic models and filling in the numerical answers by hand. There is a different logic model for each co-op. The second part of the process is completing the narrative, called the Carter-Richmond Methodology. Renee typed out each of the questions in a document, so all you need to do is fill in the answers and print it. See attached document. Please staple this document to the logic model prior to mailing it to HUD.
If you don’t have your logic model, please let me know. Alternatively, they are all available in the Shared Files under Program Staff/Service Coordination/Reporting, Evaluation and Logic Models/Logic Models. Belleville and Meyers Plaza do not have logic models, because they are budget-funded co-ops. Each Co-op only needs to submit one logic model.

Once you are done, please mail the logic model to:

HUD Detroit Multifamily Hub
Attn: Sandra Jacques
477 Michigan Avenue, 16th Floor
Detroit, MI 48226

Please mail copies to Sharon Bolden and me too. Sharon’s email is:

CSI Support and Development
Attn: Sharon Bolden
8425 E. Twelve Mile Rd, Suite 100
Warren, MI 48093

If you run into questions, you are welcome to fax me a copy for review (and email the narrative). You are also welcome to call.

Both the semi-annual reports and the logic models are due to HUD by October 30th. They do not need to be submitted in the same envelope.

Thanks all and have a great weekend,
Rachel

Semi Annual reporting of members under 62 who are frail or at risk

Answer:

I’m unsure at this point. I have some messages into HUD on this. In the meantime, Laura simply produced a quick memo indicating the number of frail and at risk members below the age of 62 with whom she is working. I think this is a good solution. I’m cc’ing the other CSI SCs so that they can utilize the same technique until we hear otherwise from HUD.

Sincerely,
Rachel

Question:

Since our Co-op has Members under 62 years of age, what additional information do we need to add in the report?  It was mentioned we needed a memo as well.  What do you want included in the memo?

PTO Procedures

New Service Coordinators:

For those of you starting on April 23rd, you are just about to enter into your eligibility for PTO. Some key things to think about when planning for PTO:

1. Your PTO is use it or loose it by the end of 2012. I encourage you to use it.

2. If you are at a Co-op with more than 1 SC, please meet with your fellow SC to arrange your PTO schedules prior to contacting me. Ideally, you should arrange your PTO so that there is always 1 SC covering in the Co-op. However, if you both would like to take time off in the same week, please try to cover the Co-op for at least a portion of that week.

3. If you are taking one week off or less, you do not need to arrange for substitute coverage.

4. Thanksgiving, the day after Thanksgiving, Christmas Eve, and Christmas Day are all holidays. You are not required to work and you do not have to take PTO for these days.

5. Please fill out a PTO request form (found in shared files) and submit to your supervisor for approval.

6. If you are sick, please call/text/email your supervisor as soon as possible in the morning to report your absence. Please call/email your liaison, Sharon, co-op president, and co-SC as well to note your absence. Submit a PTO form retroactively.

Enjoy some well-earned R&R,

Rachel
