Michigan Level of Care Screening Tool

Are you having a hard time figuring out if a resident/member might qualify for Nursing Home Care/Mi-Choice Waiver/PACE program?  They all meet the same admission requirements, and must be screened utilizing the MDCH LOC Determination Form.  This form determines if the individual meets the necessary Level of Care (LOC) requirements for these programs.  Attached is the LOC Determination form which is utilized by all organizations the offer nursing home care, medicaid waiver or PACE programming.  In addition, if you follow the link below, it will take you to the Michigan Department of Community Health (MDCH) website with more information about the LOC Determination Form and the Freedom of Choice Form.  The Freedom of Choice form must be provided to all individuals screened on the LOC Determination Form to inform them if they meet the LOC needs, plus what their options are if they meet the LOC needs.

Double Up Food Bucks Is Back!!!

The 2013 Double Up Food Bucks season starts today! Double Up Food Bucks matches the value (up to $20) spent on fruits and vegetables using a Bridge Card at participating farmers’ markets throughout Michigan.

Fair Food Network announced recently a pioneering initiative that will double the value of SNAP benefits when used for fresh produce in participating Detroit groceries. The project builds on Fair Food Network’s successful Double Up Food Bucks program, which doubles the value of SNAP benefits at farmers’ markets throughout Michigan.  The grocery project will operate in three independent stores-Honey Bee Market, Metro Foodland and Mike’s Fresh Market – between July 1 and Oct. 31, 2013. This pilot phase is expected to increase access to healthy food for 5,000 low-income Detroiters.

Wii Bowling Tournament

For those of you with avid Wii Bowlers at your Village, you may want to consider entering them in the 2013 National Senior League Will Bowling Championships being held from August 12 through October 18th.  The NSL is an internet Wii bowling league, so teams play at their own communities. No travel is involved.  There is a $150 community registration fee that allows you to enter as many teams in the tournament as you would like.  For more information follow this link: http://www.nslgames.com/.

Age of Champions Viewing

Pam asked me to forward this flyer to you about an event we are hosting. In partnership with Silver Sneakers, we are showing the documentary “Age of Champions” chronicling 5 seniors as they prepare and compete in the Senior Games, on the large screen in our theater (seats 150).

Silver Sneakers is the only organization that has rights to show the movie until August. And it so happens the 2013 Senior Games are also in July beginning the 19th.
This is a lovely, free event. As it is on a Sunday afternoon, parking will be free on the street around. And we made sure there is no Tigers game scheduled, so it wont be congested in this area.   Feel free to share!

Witness Project of Detroit

Thanks to Wylene for sharing this resource.  She had them out at Oakman Manor and reports that they were a success.

The WITNESS PROJECT®OF DETROIT   is a faith-based program that supports African American women in Detroit and Wayne County in maintaining breast health.  The Witness Project® of Detroit is a program offered through the Barbara Ann Karmanos Cancer Institute and is funded by Susan G. Komen Detroit Race for the Cure.

Locally and nationally, African American women are more likely to be diagnosed with breast cancer at later stages and more likely to die from breast cancer compared to women from all other racial and ethnic groups.

The Witness Project®  is a health education model used across the nation that was developed to promote the early detection of breast cancer among African American women through screening and cancer risk management.  The Witness Project® relies upon two types of volunteers who educate women in the community:  1) witness role models who arebreast cancer survivors who “witness” and share their personal stories about their triumph over breast cancer, and 2) lay health advisors  who are women who have never been diagnosed but provide important information about the disease and early detection.

The Witness Project® is an evidence-based intervention that is part of the National Cancer Institute’s archive of Research-Tested Intervention Programs (R-TIPS).

11th Annual Healthier Black Elders Center Health Reception

Older African Americans have much higher rates of chronic disease than older Caucasians, a disparity that the IOG has investigated for over a decade. As one means of reversing this trend and helping to make the African American community more comfortable with health interventions, the IOG created the Healthier Black Elders Reception. This reception includes inspirational and educational panel discussions with question and answer sessions, health screenings, a nutritious lunch, and exercise.

Healthier Black Elders Center- Keeping the Memories Golden: Positive Mental Health Strategies will be held Tuesday, June 11, 2013 from 7:30 am – 2 pm @ Greater Grace Conference Center.

This event will include breakfast; health screenings; sessions on fitness, humor, caregiving, nutrition, and the Affordable Care Act; and lunch.

Registration costs $5.
