Hello All! I recently encountered a problem with a MiCAFE application and received assistance from a representative and wanted to pass it your way.
I was trying to obtain access to an application that I previously was providing assistance with, but had not yet completed the application as of yet. As I was in the process of inputting the member’s name and information (to log into their application), the website indicated that the application did not exist for this particular member. The representative recommended simply inputting just the member’s social security number, instead all of the information requested. After doing so, I was able to access the application. Hope this helps! T
Hello SCs- During our peer writing workshop last week, the SCs decided to add a category to the SCoop to share documentation samples and templates. It is under “Reference”/”Templates for Documentation.” We also added a few other changes to the SCoop menus and categories, including a special area to post Hannan Senior Classes (“Company News”/”Hannan News”/”Hannan Classes”), and renamed a few of the main menu items so that they are hopefully more user-friendly.
We plan to hold a hands-on tutorial in January 2013 to allow SCs the opportunity to become more familiar and comfortable with using the SCoop. The date and time for the tutorial is yet to be determined, but it will correspond with our professional development training for the month. As always, we (Renee and Alexa) are open to any additional feedback that you may have to improve the SCoop.

Greetings All,
I’m pleased to report that Victoria Haltom, LLMSW, will be joining the Hannan Service Coordination Team on Monday, November 19th. Victoria will be working full time at Labelle Towers Co-op with Diane. I’ve attached an updated CSI SC contact list with her contact information (also on Shared Drive).
Victoria brings a creative background to the team, having worked as the Orchestra Director for two Detroit Metro School Districts, the Conductor of the Metropolitan Youth Symphony and as concert violinist for the Michigan Opera Theater! Most recently, Victoria has interned with Spectrum Human Services (including Operation ABLE) as part of her MSW program.
Please help us welcome Victoria to the Service Coordination Team. If you have an event at your Co-op or Village that you feel would be of interest as she conducts her orientation, please don’t hesitate to let me know.
Please see the attachment for a letter template drafted to a client’s physician regarding the DHS Home Help Services application. This letter was sent to a client’s physician, as they must complete a portion of the application, called the “Medical Needs Form.”
Please see the attachment for a template for writing a Medicare appeal past the initial 120 day filing period, written on behalf of a client.
(Note: The appeal from the original letter, which this template was drafted from, was approved!)
Please see the attachment for sample initial service plans, which were shared at the peer writing workshop on 11/13/12.
