Medicaid Health Plan numbers

The attached large print phone numbers are found on the back of health plan & Bridge cards. The font size makes it difficult, if not impossible for some members to read. So, this larger print hand out could help members access SNAP & health insurance information and get Dr. appointment & pharmacy transportation. You are welcome to use it and make changes specific to your local health plans!

Medicare Durable Medical Equipment (DME) policy tips

Here are some pointers for qualifying for a lift chair, and for replacing old DMEs. As you probably know already, Medicare can pay 80% and Medicaid can pay the remaining 20% for qualifying members. But, for lift chairs, there is an added cost for the chair itself.

Licensed Psychologist for Wayne County

I recently met and vetted licensed psychologist, Robert Wayne Gorman. He was referred to me by Senior Counseling Services LLC. I was looking for a counselor who took Medicare and Medicaid and in particular just members with Medicaid only. He has over 30 years of experience and specializes in substance abuse, family relations, depression and anxiety. His therapeutic approach is Humanistic, with an emphasis on spirituality. He is Caucasian. He has a private practice in Wyandotte but will come to member’s apartment for therapy. He is also available to present at a workshop on mental health issues and the benefits of counseling. He requested only referrals from Service Coordinators in Wayne County facilities. He would prefer creating a caseload between several buildings to avoid having to make single trips to facilities. Please feel free to contact him at 734-365-4119 cell or at wgorman@wyan.org

Attached you will find 3 attachments that are templates for the two standard Consent Forms that must be in each active client’s file, as well as the Confidentiality Agreement Form with the update (Hannan Service Coordination Team). I hope that this is helpful to you.
Please be sure to also electronically “file” these forms in AASC Online, so that the electronic file has an updated version of these forms without the client’s signature.
