Continuing Education Units

This program has been approved by the Michigan Social Work Continuing Education Collaborative for three (3) Continuing Education credits.  St. Mary Mercy Hospital is accredited as a provider for continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.  This program has been approved for three (3) Continuing Education credits.
Certificate of Attendance offered for activity professionals, occupational therapists, physical therapists, and speech therapists.

Educational Series for Caregivers

Turner Geriatric ClinicThe Turner Geriatric Clinic Social Work department (a part of the University of Michigan Health System) will provide an informational/educational 4-week series for those caring for aging relatives; June 5th – June 26th, 2013, Wednesday Evenings from 6PM – 8PM.

Topics that will be covered include:

  • Medication Management
  • Dementia, Delirium, and Depression
  • Legal Concerns
  • Medicare/Medicaid
  • Communication
  • Community Resources
  • Physical Changes of Aging

The series will be held at the Turner Senior Resource Center, 2401 Plymouth Rd., Ann Arbor MI.

Refreshments will be provided.

 

Registration is required. Space is limited. Please register by May 24th, 2013.

$50/person – $75/couple/family – Scholarships are available.

The presentation will provide an overview of the Federal Fair Housing Act and its impact on housing providers.  Particular attention will be paid to the applicability of fair housing law to persons with disabilities and how to identify and handle requests for accommodations. Understanding the often confusing “do’s and don’ts” of the Act is critical to avoiding infractions that can lead to unpleasant results and/or litigation.

Our panel of national experts will provide innovative models of care that are helping people make the difficult transition from hospital to home. In addition, participants will see what interdisciplinary teams from around the country are doing to address this complex problem.

After studying the information presented here, participants will be able to:

  • Identify at least three drivers of hospital readmissions.
  • Describe at least two peer-reviewed, patient-centered models for care transitions.
  • List at least three tools or lessons that will be incorporated into your own efforts to implement interventions related to care transitions.
