Ideas for Health Assessments

Hey everyone!

Henry Ford Health System has a community outreach person who is responsible for doing things like scheduling blood pressure screenings. I recently got in touch with the outreach person for Henry Ford Wyandotte, Melanie Elliott. I asked if she could help me with scheduling screenings other than blood pressure here at Flat Rock Towers. I explained to Elliott that screenings that involve puncturing skin or ingesting things are not permitted on site. Since that conversation, Elliott scheduled a therapist to come out to do grip assessments. Grip assessments tie in to balance assessments in that it tests the patient’s ability to do things like grip shower bars.

So, these are just some ideas as to how to meet those assessment criteria on the logic model. What types of things has everyone else done in their co-ops?

Logic Model Advice regarding YTD Column for CSI SCs

One key question has been how to fill out this “quarters” data and YTD on the Logic Models. If you look at the Instructions tab of your logic model, you’ll see some good answers. Everybody’s logic models are under logic models in the SC folder under program staff in shared files. However, it is a beast of the document, so the meaningful part reads:

“When reporting Activities in Year 1:

Enter your first quarter accomplishments in the “Post” column and the cumulative accomplishments in the “YTD” column. for the first quarter reporting, the number or dates will be the same in both columns. (Note: this is what you did 6 months ago).

For the second quarter of Year 1 reporting, enter the data covering second quarter activities and outcomes which occurred in that quarter only. In the “YTD” column, you will enter the cumulative total of both the first and second quarter accomplishments.”

Hence, you’ll be adding last time’s number with this time’s number for the YTD.

Logic Model Guide – Updated

For the CSI SCs in grant funded co-ops (everyone except Meyers Plaza and Belleville):

I’m attaching an updated copy of the logic model guide that Wendy compiled for your first report. I used “track changes” in red to insert a few updated suggestions. A few things to keep in mind:

1. This time you will have two columns to fill in: (a) the 2nd quarter and (b) YTD.
2. Yes, you need to do it by hand again. Let me know if you need a clean copy to work with.
3. You will likely have substantial differences from the 1st reporting period due to the swift “ramping” up of services and clients, more consistent documentation, changes in our case management procedures to reflect service provision to non-client members, and access to new numbers from the member satisfaction surveys (see individualized separate emails from me with your draft member survey results).
4. Logic model expectations are the same (with differences due solely to number of units) for all the co-ops. Hence, use each other. You are welcome to meet regionally to brainstorm. Your answers will be different based on your experiences, but the “logic” should be the same.
5. You are welcome to email/fax me a copy or request an in-person or phone meeting to discuss prior to submitting.
6. Logic models and HUD semi-annual reports are due April 30th, 2013. You should email PDFs of the semi-annual to Sheila and me, and submit hard copies of the logic models to Sheila and me. HUD needs hard copies sent snail mail (USPS) to:

The  two reports do not have to have to be submitted in the same envelope. Remember to submit one semi annual report per SC, but one logic model per co-op.

CSI SC History Lesson: Reginald Carter

A little light reading! For the CSI SCs immersed in Logic Model outcomes right now, I just wanted to let you know a bit of history! Yes, Reginald Carter asked the first seven key questions that are the foundation for the Carter-Richmond Questionnaire at the back of your logic model.

1. How many clients are you serving?

2. Who are they?

3. What services do you give them?

4. What does it cost?

5. What does it cost per service delivered?

6. What happens to the clients as a result of the service?

7. What does it cost per outcome?

CSI Logic Model Reports to HUD

There has been some confusion on the Logic Models. You will be unable to electronically open the logic models you were given. HUD has been unable to unlock them. Hence, we are proceeding by printing out the logic models and filling in the numerical answers by hand. There is a different logic model for each co-op. The second part of the process is completing the narrative, called the Carter-Richmond Methodology. Renee typed out each of the questions in a document, so all you need to do is fill in the answers and print it. See attached document. Please staple this document to the logic model prior to mailing it to HUD.
If you don’t have your logic model, please let me know. Alternatively, they are all available in the Shared Files under Program Staff/Service Coordination/Reporting, Evaluation and Logic Models/Logic Models. Belleville and Meyers Plaza do not have logic models, because they are budget-funded co-ops. Each Co-op only needs to submit one logic model.

Once you are done, please mail the logic model to:

Submitting Semi Annual Reports

Hi All,

Please submit your semi-annual reports via snail mail to Sandra Jacques at HUD at:

You should then submit your reports electronically to your supervisor and administrator (Rachel Jacobsen and Sharon Bolden for the CSI SCs).

Please remember who your audience is when writing your reports. It includes CSI or PVM as well as HUD.

Update on Logic Models for CSI SCs from Sandra Jacques. She reports that they are having difficulty with the contractor that holds the Logic Models on their sharepoint site.  We have not been able to get them opened for updating. She has asked us to complete another Logic Model Form and update with the program accomplishments so far and email it to her. I have not received this form from Sandra Jacques yet, so please start by filling out your original form out in pencil so that when we do get the new forms, you just need to type in the information.

HUD response on submitting Logic Model Outcomes

Note for CSI SCs only:

I received a couple emails back from Sandra Jacques with HUD.

Regarding Logic Models:
The contractor has to open the reports and send them to me so that I can send them to you. I am currently working on getting those open reports from them but in the event I can’t get them – I’ll just ask you to submit a new Logic Model Spreadsheet electronically that reflects the outcomes that have been achieved.

Regarding Semi- Annual Reports:

I prefer if each Service Coordinator prepares their own report.

 

I will update you as we learn more from HUD on how to submit the semi annual reports. In the meantime, please concentrate on your semi-annual report and corrective actions.

Logic Models

Wendy provided each of the service coordinators with a booklet on how to complete logic models. Additional copies of the booklet can be found on the shared drive under file name:

The logic model is a two-part document.

Each Service Coordinator has a logic model specific to his or her site; this model includes only resident data.

Additionally, each coordinator has a standard “outcomes” logic model. Consider this the “essay” part of the report. Data for this logic model includes both co-op and community residents.

Notes for completing logic models include:

  1. Field #1: Policy priority-Capacity Building and Information Sharing, Participation in, or creation of face-to-face networking meetings with other HUD Service Coordinators and local government aging and/or disability: Count each time you’ve met with other service coordinators or other service provider.
  2. Field #2: Policy priority-Capacity Building and Knowledge Sharing-Number of new strategies implemented to address service needs and increase local capacity [Strategies]: The “strategy here is defined as service coordination in your building. The answer to this field is 1.
  3. Field #9: Case Management- Reduction in move-outs to higher level of care [Residents]: Ask members “Would you have moved out if you hadn’t received this service I helped you with?”

Future Conference Calls

The conference call scheduled for August 16 will focus on storing confidential files on the shared drive, along with cleaning confidential files out of your hard drive.

To improve the impact of weekly conference calls, SCs will be writing the agenda. Agendas should be submitted to Rachel for approval prior to the meeting.

Other points of note

Think safety: Arrange your office so that you have quick access to the door in case a client becomes physically aggressive. Incidences of sexual advances made by clients should be reported to Rachel.

Best practice tip: Try to include emergency contacts on consent to release forms.

Due dates: Check requests are due every other Thursday. Rachel has added these days to her Zimbra Calendar to help SCs keep track.

Hosting a Co-op Health Fair

Information on hosting a health fair is limited and is on the logic model that each of you has for your site. You have a hard copy in your binder and it is available on the shared drive under Program Staff-Service Coordination-Site Specific (select your co-op)- Logic Model. It’s a large excel file. Open the “Year 1” tab. The last outcome/output requested in column J is:

“Policy Priority-Housing as a Platform-Number of HUD housing and community residents participating in periodic mobile health screening events  |Participants”

In order to comply with Hannan and co-op procedures, please follow the following guidelines when hosting health fair:

1. Utilize non-profit and/or governmental services (no for-profits)
2. You can provide vaccination education, but no vaccinations of any kind.
3. Check with Sharon (cc Rachel) on which neighboring co-ops you could invite in order to meet the “community” requirement. This is easy for Madison Heights – just invite New Horizons. For others, Sharon may be able to give guidance. I’m cc’ing Sharon.

Please bring your hard copies of your logic models with you to Thursday’s training here at Hannan. It will help you as Wendy shares the amazing, beastly tool she has created to help you understand and fill out your respective logic models.
