CSI Monthly SC Report – Slight Changes

I’m attaching the slightly revised monthly report format for you to submit. The only change is:

Method of posting flyer at co-op: _____________________________________________________________

The other change is that we would like you to submit your report to your liaison as well as Sheila and me. You can cc us all on the same email.

Reports are due the first week of each month and cover the previous month’s services. If you have any questions or want me to see your report prior to sending it on to CSI, please don’t hesitate to contact me.
